Consent to Services

By booking with PanelX you agree to the following. Please read it before your draw. If anything is unclear, ask us or your healthcare provider first — you can cancel any time before your draw.
1. Blood collection
You consent to having your blood (and, for some tests, urine) collected by a trained phlebotomist at your home, or by staff at a lab location you choose. Blood draws are routine and low-risk, but can cause:
- Bruising, soreness or slight bleeding where the needle went in
- Feeling lightheaded or fainting
- Rarely, infection, a blood clot, or irritation of a nerve
- Needing more than one needle stick to collect enough blood
Tell your phlebotomist if you have fainted during a draw before, take blood thinners, have a bleeding disorder, or are allergic to latex or adhesive. You can stop the draw at any time.
2. Before your draw
Some tests need preparation, such as fasting or pausing certain supplements. We send these instructions when you book and again the evening before. Before drawing, your phlebotomist will confirm your identity, ask whether you followed the instructions, and ask you to sign to confirm your answers. If you did not follow them, your results may be less accurate; you can choose to go ahead or reschedule.
3. Testing and results
- Your samples are tested by CLIA-certified partner labs.
- Where required by law, a licensed clinician orders your tests.
- Your results are delivered to your PanelX account. They are not a diagnosis. Discuss them — especially any outside the normal range — with a healthcare provider.
- If a result could need urgent attention, we or a clinician may contact you by phone.
- Occasionally a sample cannot be tested (for example, if it is damaged in transit). We will offer to redraw it.
4. Physician review
If you add a physician review, you consent to that physician seeing your results and contacting you about them. The review is a consultation about your results, not ongoing care.
5. Household visits and children
- Each adult on a household visit consents for themselves and receives results only in their own account.
- If you book for a child, you confirm you are their parent or legal guardian, consent to the draw on their behalf, and will be present during it.
6. Specialty tests
Some tests require a prescription or have their own consent form. We will tell you before your draw if a test needs either.
7. Emails and texts
You agree that we may email and text you about your booking — confirmations, preparation instructions, arrival windows, reminders and when results are ready. Message and data rates may apply. Reply STOP to any text to stop texts; booking emails will still be sent.
8. Your information
How we use and protect your information is explained in our Privacy Policy.
